Over the period of 2012-2014, Dr. Sorensen sought to build on the collective experience of Minnesota health systems invested in the delivery of medication management services, facilitated inter-organizational collaboration and disseminated knowledge and experience that highlighted the impact of these services.

Description: The project presents the evolution of medication management services and factors influencing success in integrating these services in six health systems in Minnesota. The analysis explores how medication management has maintained its presence as a priority service and received continued organizational support across these systems. Finally, key lessons for peer organizations on integrating medication management is presented.

Improving Transitions of Care through Comprehensive Medication Therapy Management
Description: The goal of this effort is to improve medication management during care transitions from the acute to ambulatory patient care settings by: 1) applying the IHI Model for Improvement to engage patients in comprehensive medication management services in the ambulatory care setting within 7 days post-hospital discharge, 2) evaluating drug therapy problems identified post-discharge, and 3) collaborating with inpatient teams to facilitate improvements in the discharge process.

A Pilot Study to Evaluate the Impact of Health Record Access on Drug Therapy Problem Determination by Pharmacists
Description: This evaluation reviewed the differences in the number, type, and confidence in drug therapy problems (DTPs) identified by pharmacists with and without access to a primary care health record and to determine what information was needed by pharmacists to confirm accuracy of their DTP assessment. The study design was an exploratory, comparative evaluation of comprehensive medication management assessments.

We employed thematic analysis to examine the decision making process and influential factors through open-ended, semi-structured interviews.
