Site Profile
The St. Cloud VA Health Care System is a Joint Commission-accredited health care institution providing outpatient care in the areas of internal medicine, psychiatry, cardiology, pulmonology, rheumatology, neurology, nephrology, urology, home care, hematology/oncology, podiatry, pain, orthopedics, and urgent care. Inpatient care is provided in long-term care, hospice, acute psychiatry, and residential rehabilitation.
Pharmacy Services: Medication Therapy Management and collaborative prescribing are provided by over 20 Clinical Pharmacy Practitioners for both outpatients and inpatients via referrals for management of anticoagulation, hypertension, diabetes, pain and palliative care, and other chronic conditions for thousands of patients annually. Automation includes provider order entry, completely computerized medical record, bar code medication dispensing and administration, and automated dispensing cabinets.
Educational Experiences
Each resident gains exposure to many areas of direct patient care in a team-based setting by providing care via 30-50 patient encounters per week. Our pharmacists have a scope of practice which allows for medications to be independently managed by the pharmacist through provider referral and population management, and includes prescribing privileges. The resident will work collaboratively with the interprofessional health care team at this site including physicians, nurse practitioners, physician assistants, psychologists, nurses, dieticians, social workers, scheduling staff and pharmacy technicians. Residents will complete concentrated learning experience rotations in the areas of primary care, anticoagulation, cardiology, pain, mental health, long-term and rehab care, and management.
In our primary care area, the resident will serve as the pharmacist on the Patient Aligned Care Team (PACT). As the PACT pharmacist, the resident will be responsible for meeting with veterans for face-to-face visits, conducting telephone encounters, virtual visits, completing medication consults, responding to drug information questions, and participating in team meetings. Typical disease states encountered include anticoagulation, diabetes, dyslipidemia, hypertension, hypothyroidism, heart failure, and other chronic disease states. In PACT, each appointment is 15-30 minutes in length, during which the resident reviews and reconciles the patient's medications, assesses vital signs, evaluates laboratory results, and formulates a treatment plan. A progress note is entered into the electronic chart for each visit. The resident is also responsible for scheduling appointments, ordering labs, ordering medications and conferring with a provider when questions arise.
Specialty practiceexperiences in the areas of pain, cardiology and mental health function similarly as PACT with integration into the clinical practice teams . This experience provides the resident broadened practice exposure and therapeutic treatment planning working with preceptors with advanced training and experience in these care modalities.
Our Community Living Center (CLC) provides a nursing home level of care and a variety of rehabilitation programs. The resident will be exposed to a variety of patients admitted to CLC, including rehab, long-term stay and hospice patients, through interviewing of patients, chart review and consultations from providers. In this area, the pharmacy team is responsible for medication reconciliation on admission, discharge counseling, monthly nursing home chart reviews, pharmacy consult completion, fall reviews, drug information questions, and disease-state management. CLC chart review functions encompass comprehensive MTM reviews of our veterans on a monthly basis. Through chart reviews, the pharmacy resident will gain experience evaluating a variety of geriatric disease states including dementia, Parkinson’s, osteoporosis, BPH and others, as well as learn the management of other chronic disease states in a geriatric population. The resident will be responsible for disease state monitoring in the area of anticoagulation, diabetes, hypertension and antimicrobial monitoring. The resident will assist with managing transitions of care between settings through admission medication reconciliation, plus discharge education and outpatient appointment scheduling.
or our practice management experience, the resident will be exposed committee involvement, policy/protocol review and development, medication safety projects, quality improvement projects, population pharmacotherapy, leadership development, organizational learning, and introduction to pharmacy management roles. The resident will be involved in the medication error committee and the P& T Committee. The resident will complete a performance improvement project and medication use evaluation.
Education and teaching experiences are provided through regular presentations of journal club, topic review, and case presentation opportunities to our pharmacy staff. During the second half of the residency, each resident will provide preceptorship for APPE students and teaching assistant for the University of Minnesota. A longitudinal residency project will be completed throughout the residency year.
Traditional pharmacy staffing duties will be expected approximately one day of every 2 weeks, in many of our care areas, depending on the rotation and the learning needs of the resident. The resident will also have residency time each block to work on projects, drug information questions, presentations and other assignments.

J.D. Anderson, PharmD, BCPS is the Chief of Pharmacy at the Saint Cloud VA Health Care System, a role in which he’s served since 2016 after serving 5 years as the Associate Chief of Pharmacy. From 2004-2011, he was a Clinical Pharmacy Specialist in the facility’s outpatient clinics. He is a graduate of the University of Minnesota College of Pharmacy in 2000 and completed a residency in Paynesville, MN through the University of Minnesota Ambulatory Care Residency Program in 2001. He is a past recipient of the University of Minnesota College of Pharmacy’s Student Preceptor of the Year and Minnesota Pharmacists Association (MPhA) Innovative Pharmacy Practice Awards.

Stephanie Cone, Pharm.D., BCPS is a Clinical Pharmacy Specialist at the St. Cloud VA Health Care System, and was our first residency graduate in 2006. She is a graduate of South Dakota State University College of Pharmacy. Dr. Cone provides patient care in a Patient Aligned Care Team (PACT)and serves as a preceptor for pharmacy students and residents.
Ryan G. Davis, Pharm.D., BCPS is a Clinical Pharmacy Specialist at the St. Cloud VA Health Care System. He is a graduate of the University of Wisconsin - Madison School of Pharmacy in 2003 and completed a primary care residency at the Carl T. Hayden VA Medical Center in Phoenix, Arizona in 2004. Prior to joining the St. Cloud VA in 2019, he was a Clinical Pharmacy Specialist at the Denver VA Medical Center and the Golden VA Clinic in Colorado from 2004-2018. Dr. Davis provides patient care in a Patient Aligned Care Team (PACT) and serves as a preceptor for pharmacy students and residents.
Jana Zeiher, Pharm.D., BCPS, CDE is a Clinical Pharmacy Specialist at the Saint Cloud VA Health Care System. She is a graduate of the South Dakota State University College of Pharmacy and completed a residency in Paynesville, MN through the University of Minnesota Pharmaceutical Care Residency Program. Dr. Zeiher provides patient care in a Patient Aligned Care Team (PACT) and assists with treating patients with Hepatitis C. She serves as a preceptor for pharmacy students and residents.
Allison Gilmore, Pharm. D. BCPS is a Clinical Pharmacy Specialist at the St. Cloud VA Health Care System. She is a graduate of Campbell University College of Pharmacy and Health Sciences in North Carolina and completed a residency at the St. Cloud VA. Dr. Gilmore provides patient care and precepts residents in the Community Living Center.
Tori Ziska, Pharm.D., BCACP,is a Clinical Pharmacy Specialist with the pain clinic at the Saint Cloud VA Health Care System. He graduated from North Dakota State University in 2015 with a Doctor of Pharmacy. Gerard completed a PGY1 at Avera McKennan Hospital and University Center in Sioux Falls, South Dakota and then a PGY2 specializing in psychiatry at Avera Behavioral Health also in Sioux Falls. He started working with the Saint Cloud VA in June of 2017 as the pharmacist provider building the pain clinic practice. In 2020, he transitioned into mental health and has assisted with further practice expansion to include substance use disorder.

He graduated from North Dakota State University in 2015 with a Doctor of Pharmacy. Gerard completed a PGY1 at Avera McKennan Hospital and University Center in Sioux Falls, South Dakota and then a PGY2 specializing in psychiatry at Avera Behavioral Health also in Sioux Falls. Sara Olson, PharmD is a Clinical Pharmacy Specialist at the St. Cloud VA Health Care System. She graduated from Drake University with a Doctorate in Pharmacy in 2019. Dr. Olson completed her PGY1 Pharmacy Residency at the Kansas City VA Medical Center in Kansas City, Missouri. She then went on to complete a PGY2 Ambulatory Care Pharmacy Residency at the Captain James A. Lovell Federal Health Care Center (FHCC) in North Chicago, Illinois. At FHCC Dr. Olson had the unique opportunity to provide care for both veterans and active duty service members at this combined VA and DoD facility. Upon completion of her residencies, she started working with the St. Cloud VA in August of 2021. Dr. Olson provides patient care and precepts residents in the patient aligned care team (PACT).
Katie Klepel, PharmD is a Clinical Pharmacy Specialist in Mental Health at the St. Cloud VA Health Care System. She graduated from Drake University with a Doctorate in Pharmacy in 2018. Katie then completed a PGY1 Pharmacy Residency at the Central Iowa VA Health Care System in Des Moines, IA. Dr. Klepel precepts residents in the outpatient mental health clinic and inpatient acute psychiatric unit.
Kristen Schroeder, PharmD is a Clinical Pharmacy Specialist at the St. Cloud VA Health Care System. She is a graduate of South Dakota State University and completed a residency in 2019 at the St. Cloud VA. Dr. Schroeder provides patient care and precepts residents in the pain clinic, as well as in a patient aligned care team (PACT).
Jessica Trompeter, PharmD, MBA, is the Associate Chief of Pharmacy at the Saint Cloud VA Health Care System. She is a graduate of the Bernard J. Dunn School of Pharmacy at Shenandoah University. She completed a PGY1 Pharmacy Practice residency at Valley Health Care System and completed a PGY2 residency in Primary Care at Amherst Family Practice in Winchester, VA. After residency completion, she was an Assistant Professor at Shenandoah University in the Pharmacy Practice department and Division of Physician Assistant Studies. Jessica has worked at the St. Cloud VA since 2012 and previously provided patient care in the Extended Care and Rehab service at the St. Cloud VA. She oversees the Educational experience in the residency program and assists with direct precepting on an as needed basis.
