Continuing Education: Advancing Kidney Health through Optimal Medication Management

Do you, anybody in your family or a friend have diabetes, high blood pressure or kidney disease? One in three American adults are at risk for kidney disease. Diabetes and high blood pressure are the leading causes of kidney disease. 37 million American adults (1 in 7) have kidney disease. Dr. Wendy St. Peter, PharmD (UMN College of Pharmacy faculty) is directing the national Advancing Kidney Health through Optimal Medication Management (AKHOMM) initiative to stamp out kidney disease! Dr. St. Peter and several other faculty, staff, graduate and professional pharmacy students are driving practice transformation in medication management for persons with chronic kidney disease (CKD). AKHOMM has

created an ongoing online curriculum to educate pharmacists and other health care practitioners on how to prevent and manage CKD by optimizing medication management. They are pairing this with a Learning and Action Collaborative to help practices integrate a pharmacist into their team to provide comprehensive medication management.
