Geritom Med, Inc. is a Long Term Care pharmacy located in Bloomington, MN that succeeds by delivering outstanding service to a wide array of long term care customers, including Intellectual and Developmental Disability patients, Intermediate Care Facilities, Adult and Child Foster Care, Assisted Living Facilities, Senior Care facilities, and some individual customers with complex and chronic health conditions. Educational Experience

The resident can expect to participate in direct patient care three days a week conducting MTM visits, one day staffing, and one day on projects. They will be providing comprehensive medication reviews, documenting recommendations, and communicating with prescribers and facility staff. These visits take place at the facilities where the individual patients reside which will include driving from Geritom to these facilities. A Geritom vehicle will be provided for this purpose on MTM days. There will be a large exposure to Psychiatry and Neurology, among other disease states at this practice location. There is an emphasis on the use of pharmacogenomic testing (PGx) within the population being served and the resident will utilize PGx as a tool to care for these patients. As the year progresses the resident will have more opportunity to work with minimal supervision.

The resident will work closely with their preceptor(s) with regular opportunities to review patients and ask questions. Other opportunities include journal clubs, addressing drug information questions, staffing, consulting, and completing a residency research project. The resident will also participate in Academic Days and participate in the residency program’s Curbside Consult publication.

Dr. Jason Wachtl, Pharm.D., BCACP, BCGP,  is an experienced executive leader with a strong focus on management, operations, and compliance of Pharmacies with over 20 years of experience in the field. Jason currently serves as Vice President, Pharmacy and Clinical Programs at Geritom, a large independent Pharmacy where he oversees all compliance, accreditation, clinical, and pharmacy programs. Jason’s past experience includes running Medication Therapy Management Programs at the district level and Pharmacy Management at a large retail Pharmacy chain. Jason has earned a Doctor of Pharmacy from University of Iowa in 2007 and is currently a Board Certified Geriatric Pharmacist (BCGP). He lives in Maple Grove, MN and enjoys sports, cars, grilling, music, and spending time with his family in his free time.
