Site Profile

Allina Health is a not-for-profit health care system that cares for patients from beginning to end-of-life through its 61 clinics, 13 hospitals, 15 retail pharmacies, 23 hospital based clinics, and 49 rehabilitation locations. We provide services in the Twin Cities metro area and various greater Minnesota and Wisconsin locations. In 2012 Allina Health was selected as a Pioneer Accountable Care Organization by the Center for Medicare Services. 

Allina Health CMR pharmacy services

Allina currently has six metro pharmacists that provide Comprehensive Medication Review services to patients. These pharmacists see patients in 17 clinic locations via referral and work under multiple Collaborative Practice Agreements with providers.  Allina pharmacists are also part of an interdisciplinary team of clinicians comprised of the pharmacist, RN, social worker and non-clinical care guide. This team is focused on improving health during transitions of care and reducing hospital readmissions. 

Educational Experience

A majority of the resident’s time will be spent working directly with patients conducting comprehensive medication reviews, documenting these encounters and communicating recommendations with providers. These visits may take place in clinic or in the patient’s home. The resident will have the opportunity to interact with many different members of the health care team, as they will be closely aligned with the interdisciplinary care management team and may conduct co-visits with some of these clinicians. The resident will work closely with their preceptor(s) with daily opportunities to review patients.

The resident will also have the opportunity for other activities to include participating in journal club, answering drug information questions, participating in care management and provider meetings, and completing a residency research project. The resident will also participate in Academic Days and Residency Seminars and have the opportunity to teach through the University of Minnesota, College of Pharmacy.

As the year progresses the resident will have more opportunity to work with minimal supervision, and possibly to establish a practice site at a new clinic location.

mckaya photoMckaya Kastner, PharmD, BCPS is 2013 graduate of North Dakota State University. After graduation, she completed an ambulatory care residency at St. Joseph Regional Medical Center in Mishawaka, Indiana. Mckaya is a Board Certified Pharmacotherapy Specialist and has been a pharmacist practitioner with Allina Health since 2015. Her interests include transitions of care, medication adherence and process improvement. Mckaya sees patients at the Plymouth, Richfield and Nicolet Mall clinics, and completes home visits in Minneapolis and the surrounding west metro region.  



nathan photoNathan Blake, PharmD has worked at Allina as an ambulatory pharmacist practitioner since 2015. He graduated with his PharmD degree from the University of Minnesota in 2004. Nathan started a Pharmaceutical Care practice at the University of MN Physicians Primary Care Clinic following graduation and later pioneered the Medication Therapy Management (MTM) Program at Target Pharmacy. Nathan now service as the Director of Ambulatory Care Management.



Ann photoAnn Byre, PharmD has worked at Allina Health since 2009, and is the system vice president for pharmacy services across the continuum. She earned her PharmD degree from South Dakota State University in 2001 and began her career as a pharmacist with Target Corporation before coming to Allina Health. In her leadership role, Ann is passionate about mentoring students and residents and offers APPE, PGY1 and PGY2 rotations that give a glimpse into health system pharmacy leadership.



Anjanette photoAnjanette Finnegan, PharmD has worked at Allina as a pharmacist practitioner since 2015. She earned her PharmD degree from Creighton University School of Pharmacy in 2013 and completed a PGY-1 Ambulatory Care Residency at Community-University Health Care Center in Minneapolis, Minnesota. She sees patients at Allina Cottage Grove, Hastings, and Faribault clinics, and completes home visits in the southeast metro region.



Kressel photoMatt Kresl, PharmD, BCPS has worked with Allina Health since 2004. He graduated with his Doctorate of Pharmacy degree from the University of Minnesota in 2004. He completed a pharmacy practice residency at Abbott Northwestern Hospital in 2005. He then worked as an acute care clinical pharmacist and manager at Abbott Northwestern from 2005-2015. He sees patients at the Allina E. Lake St., Bloomington, and Shakopee clinics for patients in the west and south metro region.



mORK PHOTOLee A. Mork, MS, RPh, MBA is Director of Ambulatory Pharmacy Services for Allina Health. In his role, he is responsible for the clinic’s MTM program, pharmaceutical purchasing, a patient medication assistance program and e-prescribing prescription technologies for 72 primary care clinics. He received his Bachelor of Science in Pharmacy and Master of Science in Hospital Pharmacy from the University of Minnesota and received a Master of Business Administration from the University of St. Thomas. He completed hospital pharmacy administration residency at University of Minnesota Hospital and Clinics in Minneapolis Minnesota. He has worked for Allina Health since 2007.


Jonica photoJonica Thorson, PharmD, BCACP graduated from the University of Minnesota in 2010 and completed an ambulatory care residency with Fairview. She started working at Allina in 2012 and currently sees patients at the Bandana Square, Maplewood, and Shorview clinics and sees patients in their homes in the northeast metro region.
