Bringing Practitioners Together to Share Successes and Inspire Action in Practice

"Mobilizing Practice Transformation in Medication Management for the Triple Aim" was held Nov. 12-14, 2014 at the Radisson Blu at Mall of America.

The event brought together 175 practitioners, administrators and students from 23 states to share successes and inspire action in practice transformation focused on optimizing medication use.

“The event was nontraditional in that it was designed to produce action, not just offer educational sessions,” said Professor Todd Sorensen, who led the planning of the event. “Participants were engaged in interactive sessions and were provided time to reflect and plan. Many participants left the meeting with a set of action items focused on enhancing the medication management services they provide within their home practice setting.”

Speakers included Dennis Wagner, CMS Innovations Center; Mark Loafman, Feinberg School of Medicine, Northwestern University; Andrea Walsh, HealthPartners; and Allyson Schlichte, Fairview Pharmacy Services.

The conference was hosted by the college’s Department of Pharmaceutical Care & Health Systems and the Peters Chair for Pharmacy Practice Innovation, Alliance for Integrated Medication Management, the U’s Ambulatory Care Residency Program, and the UPlan MTM Network.
