MN Health Plan & Provider Initiative

Purpose

The College of Pharmacy convenes Minnesota health care plans and pharmacist care providers to spark dialogue and improve health outcomes. We intend to optimize medication outcomes for the citizens of Minnesota and beyond while improving health across the state through better medication management. Through gathering stakeholders, we aim to drive optimal value and payment for Medication Therapy Management (MTM) and other pharmacist services provided and foster collaborations and connections between payers (health plans) and providers (pharmacists) of MTM and other services. Impact

Through this initiative, we have increased transparency between pharmacist care providers and health plan leaders, leading to increased collaboration and network building. These conversations have also increased the understanding and appreciation from both plans and pharmacists that we all share the same goal: to achieve the best health outcomes for our patients. Previous work and open communication has prepared pharmacy in our state to successfully launch broader payment for pharmacist services, which is being implemented in 2025.
