CentraCare Health is a not-for-profit healthcare system that provides comprehensive, high-quality care to people throughout central Minnesota and is comprised of St. Cloud Hospital, five regional critical access hospitals, and 17 regional clinic sites. St. Cloud Hospital is a 489 bed tertiary care facility with sole community hospital status. There are approximately 28,000 hospital admissions and 260,000 outpatient visits each year.

Pharmacy Services: CentraCare Health – St.Cloud began providing comprehensive medication management services in 2010 in the family practice setting. In 2013, a full-time pharmacist was designated to facilitate transitions of care from the hospital to home setting. Starting in 2014, CentraCare Health – Family Practice at the Health Plaza hired a pharmacist to join their TEAM care model, now Chronic Disease State Management, in which the pharmacist worked alongside other healthcare professionals. In 2016, ambulatory pharmacy services expanded to the Internal Medicine Clinic. Ambulatory pharmacy services joined the Integrated Pain Medicine Clinic in 2017.
CentraCare Health pharmacy services include three community clinic pharmacies, decentralized and centralized inpatient pharmacy services, infusion pharmacy, and an operating room pharmacy. Additional pharmacy services throughout the health system include adult immunizations, tobacco cessation counseling, diabetes education, asthma management, medication reconciliation, warfarin management, kinetics, cardiac rehab medication education, and drug information and nutritional support.
The residency will be centered upon developing clinical skills and learning to care for patients through comprehensive medication management. In addition, the resident will be able to assist with the continued development of comprehensive medication services for CentraCare Health. Practice sites for the resident may include Internal Medicine, Family Health Center, and Family Medicine. Other potential opportunities include collaborating with the nephrology and cardiology clinics. The expectation for the resident will be to work collaboratively with the clinic care team to provide comprehensive medication management as well as provide focused drug information. Additional opportunities for the resident include working with the transition of care pharmacist to identify potential patients who may benefit from ambulatory pharmacy services. By the completion of the residency year, the resident will learn the skills necessary to build a successful ambulatory pharmacy practice.
The resident can expect to meet with patients for face-to-face medication management services 3 to 4 days of the week. Additional time will be devoted to project management, preparing for presentations, and completing activities set forth by the University of Minnesota. Pharmacy staffing will be included as part of the resident’s learning experience.
Jessica Lind, PharmD, BCACP, is a clinical ambulatory pharmacist for CentraCare Health River Campus, Internal Medicine Clinic. She graduated from the University of Minnesota- College of Pharmacy in 2011 and from Minnesota State University, Mankato in 2007 with a Bachelor of Science degree. She completed a PGY-1 residency at FirstLight Health System in Mora, MN and a PGY-2 ambulatory practice development residency at Hennepin County Medical Center. Jessica began providing care for patients at CentraCare Health – Paynesville in 2013, following completion of her residency programs. In 2016, she transitioned her role to the Internal Medicine clinic to assist with the continued development and expansion of the ambulatory pharmacy department in St. Cloud. Other past pharmacy experiences include community pharmacy, long-term care, and oncology pharmacy. Currently, she is working to develop comprehensive medication management services at the Internal Medicine clinic. Jessica is a past member of the Board of Directors for the Minnesota Pharmacists’ Association. Her pharmacy interests include ambulatory pharmacy practice development and rural pharmacy practice.
Michael F. Akers, PharmD., is a clinical pharmacist at St. Cloud Hospital where his main roles include serving as the transitions of care pharmacist along with working on a multidisciplinary heart failure service. He has been working at St. Cloud Hospital since 2012 and worked in various areas prior to developing and implementing the transitions of care pharmacist position. Dr. Akers studied biology as an undergraduate at the University of Minnesota prior to receiving his Doctor of Pharmacy degree (Pharm.D.) from the University of Maryland-Baltimore. He then went on to complete a PGY-1 pharmacy residency through the University of Minnesota’s Ambulatory Care program with his practice location being at Essentia Health. Dr. Akers serves as an Advanced Pharmacy Practice Experience (APPE) preceptor for University of Minnesota College of Pharmacy students as well as North Dakota State University. His interests include: continued development of transitions of care pharmacy services, cardiology with a focus on heart failure, geriatrics, research, and teaching.
Keith Karsky, RPh, serves as the Coordinator of Retail Pharmacy Services for CentraCare Health and has been with CentraCare since 2001. Keith has responsibility for CentraCare’s three retail pharmacies, with an emphasis on growth and pharmacist and program development, such as medication therapy management and pharmacist administered immunizations, allowing pharmacists to utilize their pharmacy expertise. Prior to his current position, Keith was the Pharmacist in Charge at CentraCare Pharmacy Health Plaza. Keith earned his pharmacy degree from North Dakota State University and is a registered pharmacist and preceptor in the state of Minnesota.
