Fairview is a not-for-profit healthcare system in the metro area of Minneapolis/St. Paul as well as other areas in greater Minnesota. Fairview has 7 hospitals, 36 primary care clinics and 55 specialty clinics. Fairview has over 22 retail outpatient pharmacies.
Pharmacy Services:
Fairview has provided medication therapy management (MTM) services in their clinics since 1998. We currently have 12 different MTM locations. Eleven of these are in Fairview clinics; one is in a retail pharmacy location. Our MTM pharmacists have cared for over 7,000 patients and identified over 25,000 drug therapy problems.
Other Fairview Pharmacy Services areas include: retail pharmacy, hospital pharmacy, home care, specialty pharmacy, home infusion pharmacy, clinical trials and Fairview’s own PBM, Clearscript.
Resident’s Educational Experience:
The resident will work collaboratively with the medical staff (MDs, CNP, and nursing staff) to address patient’s medication related concerns. The resident is responsible for all aspects of MTM including:
In addition to providing MTM services, the resident will be available to answer drug information questions (dosage recommendations, formulary medications, etc.) and research medication topics for the providers.
A typical work week includes two full days at Crosstown and two full days at another MTM site. The rest of the time is devoted to preparing presentations and lectures, working on the research project, attending and leading group education sessions, precepting students, and completing activities set forth by the University of Minnesota. The resident will attend and participate in "Residency Seminars" and “Academic Days” as determined by the residency program.
Site Coordinator:
Amanda Brummel, PharmD has been with Fairview since 1999, and has practiced MTM for over 7 years. In that time, she has provided MTM in 5 different practice locations. She is now the manager of the MTM program, but continues to practice at the Fairview Uptown Clinic. She teaches a class at the University of Minnesota titled “How to Build an MTM practice”.
Additional Preceptors:
Preceptor Molly Ekstrand, RPh has been with Fairview since 2002. She has practiced MTM in both retail and clinic settings. She has practiced at 4 different locations at Fairview. She recently took over the Crosstown clinic as a practice location. Molly enjoys teaching and working with students.
