Organization:
Saint Elizabeth’s Medical Center (SEMC), located in Wabasha, Minnesota, has been serving the health care needs of eastern Minnesota and western Wisconsin residents for more than 107 years. A member of Ministry Health Care and sponsored by the Sisters of the Sorrowful Mother, SEMC is licensed for 31 beds and is a Critical Access Hospital (CAH). Our facility is Medicare certified for 25 acute care/swing beds, 100skilled nursing home beds, and 48 assisted living apartments. SEMC’s mission is to continually improve the health and well-being of all people, especially the poor, in the communities we serve. This is accomplished by offering a broad range of acute care, diagnostic, educational, emergency, long-term care, outreach, and specialty services to residents within our service area.
Pharmacy Services:
St. Elizabeth’s pharmacy provides a broad range of services to both acute care and LTC patients. We work closely
with our physicians in the selection, dosing, and monitoring of medications including warfarin, aminoglycosides,
and TPN. Other services include medication administration record management, drug use evaluation, community,
staff, and disease management education, and participation in QI teams. Our pharmacists have a special interest in
patient safety initiatives. Pharmacists are on-call to provide after hours services for both acute care and LTC.
Resident’s Educational Experience:
Key learning activities and functions during this residency will include assisting in the development and staffing of Medication Therapy Management services, and participating in the interdisciplinary care of disease management in hospitalized patients, long-term care residents, and employees participating in SEMC’s wellness program.
Site Coordinator:
Kurt graduated from the U of MN Pharm.D. program in 1995. Since that time, he has served as the Director of Pharmacy at St. Elizabeth Medical Center. He is an active member in various in-house teams including Pharmacy and Therapeutics Committee, Medicine/ER Team, Infection Control, and OB Care Team. Kurt is involved with patient and community education in SEMC’s cardiac rehab and pulmonary rehab programs as well as our diabetes management program. He also participates in our Medical Staff meetings and Ministry Health Care Pharmacy Value Analysis Team. His interests include cardiovascular therapy and infectious diseases.
Additional Preceptors:
Tracy graduated from the U of MN Pharm.D. program in 1995. From 1996 to 1998, she worked as a staff pharmacist at Red Wing Corner Drug in Red Wing, MN. She was also their site coordinator for APhA’s Project ImPACT. She has worked at SEMC since 1998 as a staff/clinical pharmacist. She is actively involved in various in-house teams and committees, including OB Care Team, Infection Control Team, Medication Variance Team, Medicine/ER Team, Medication Reconciliation Committee, and Forms/Core Measures Committee, and Wellness Committee. She is also an integral participant in SEMC’s USP 797 and Hazardous Waste Compliance Initiatives.
