Application Deadline: January 5, 2009

Eligibility: Doctor of Pharmacy degree (Pharm.D.), eligible for immediate licensure by the Minnesota Board of Pharmacy.

Criteria for Selecting Residents: Residents are selected based on their commitment to patient care as well as their dedication to advancing pharmaceutical care practice. Demonstrated involvement in a variety of professional activities is highly desirable.

• Cover Letter: Describe reasons why you are applying to this residency program. In addition, include the specific residency experience(s) within our program for which you are applying.
• Curriculum Vitae
• Academic Transcript (NOTE: Transcript(s) may be sent by regular mail.)
• Recommendations: Provide three recommendations from individuals with whom you have a professional relationship. For applicants graduating in Spring 2009, at least one of the references should be from a faculty member at the college or school of pharmacy from which you will graduate. (NOTE: References should also be submitted electronically to pharmres@umn.edu.)
