Immunization Policy


Immunization Policy

All Academic Health Center students, including those in the Pharm.D. program in the College of Pharmacy, are required to submit an immunization form to Boynton Health Services indicating the following vaccinations:

  • Mantoux Tuberculin (A two-step Mantoux is required for the first year and a one-step Mantoux is required each year after that.)
  • Measles/Mumps/Rubella (MMR)
  • Hepatitis B (3 step—must have at least 2 of 3 shots to enroll)
  • Varicella (A patient reporting a history of the disease is considered adequate proof of immunity. If the patient is uncertain or reports not having had chicken pox they must show proof of immunization.)
  • Tetanus/Diptheria

In order to request your immunization records to be forwarded to a third party (such as a preceptor or clerkship site), students must visit Boynton Health Services in person. Students can either meet with Ruth Aldana in her office (Boynton #223, between 8:00 - 4:00 M-F) or at the Boynton front desk, where they must fill out an information release form.
