Pharm.D. Requirements
CPR Training:
The University of Minnesota College of Pharmacy requires all Pharm.D. students to have valid CPR certification upon entering the Pharm.D. program and to maintain that certification through graduation from the program. This certification must be either Basic Life Support for Health Care Providers (American Heart Association—two‐year certification) or CPR for the Professional Rescuer (American Red Cross—one‐year certification). This qualification ensures all students have demonstrated the skills necessary to respond during emergencies while participating in Early, Introductory, and Advanced Pharmacy Practice Experiences (EPPEs, IPPEs, APPEs) and internships. The Office of Student Services verifies all certificates of CPR training completion. Students who do not have current certification will have a registration hold placed on their record and will not be allowed to participate in required EPPEs, IPPEs, and APPEs.

Immunizations:
Students can see their immunization status on the MY U portal (
www.myu.umn.edu), click on "MyU Space." Please contact Boynton Health Service for questions regarding the AHC immuniation policy. Visit the
Boynton Health Service website to access the immunization form. Students who are not in compliance with the AHC immunization policy will have a registration hold placed on their record. Boynton offers a
monthly walk-in TB skin test clinic, free to AHC students.

Mandatory Health Insurance for AHC Students
All pharmacy students will be automatically enrolled in Boynton Health Service's health insurance for AHC students unless they provide certification proving they are covered by adequate health insurance. Please visit Boynton's website

HIPAA Training All pharmacy students are required to complete a series of training modules including Health Insurance Portability and Accountability Act (HIPAA) training. This is part of a broader online module entitled "Public Jobs, Private Data" which encompasses aspects of information privacy. Students can complete this training online - please visit

Background Checks
In accordance with state law, the College of Pharmacy performs criminal background checks on all enrolled students each year. Students who fail the background check may be prevented from curricular activities involving patient contact. Please contact the
Minnesota Department of Health for more information on criminal background checks.
How to share with your preceptor: Please save the confirmation you received in the mail from the MN Dept. of Health. If you have lost this form, please contact the Office of Student Services to request a copy to be sent to your preceptor.
Internships The state of Minnesota requires students obtain 400 hours of internships (in addition to 4th yr. APPEs) in specific duties before becoming licensed as a pharmacist. Please consult the
Minnesota Board of Pharmacy for more information.
