Teaching Philosophy

It is our goal when serving out patients to deliver the highest level of pharmacist care possible. Progressive Health Care recognizes the value of pharmacists in finding, fixing and preventing drug therapy misadventures. Students will be challenged to work with our staff and assist the pharmacy staff in providing this level of care. Students will work first hand with patients and nursing staff to refine infusion therapy for each individual patient. The student will be expected to work with multiple patients on different services from admission through discharge.

Progressive Health Care is an infusion pharmacy providing multiple intravenous therapies for patients within a 100 mile radius of Bemidji. The services provided by this site include: biologicals, chemotherapy, aminoglycoside dosing and monitoring, enteral nutrition, antibiotic dosing and monitoring, total parenteral nutrition and pain management. The patients we serve are located at home, in nursing homes or occasionally in a hospital. Progressive Health Care works closely with 13 nursing agencies in northern Minnesota and 13 long term care facilities.

Progressive Health care of Bemidji is a company that strives to ensure that the people of Northern Minnesota receive the highest level of pharmacists care possible in dealing with conditions that require parenteral or enteral therapy. We work with our patients as part of their health care team to provide convenient, timely and professional care that will minimize disruption to the patient and their family and maximize therapy outcomes.

Pharmacy Services

Professionalism and Assessment
Students will meet with their preceptors most weeks to discuss patient cases, expectations, and answer questions.

Patient Care – Home infusion

APPE Structure
The student will begin the rotation with an IV skills review. This includes compounding medications, training in central and peripheral lines, and review of different pump options for patients at home and in nursing homes. Working loosely with our technical and support staff will be important in this portion of the rotation. Once that is completed the student will work directly with a pharmacist on new referrals/patient admissions. The student will be assigned patients as they are admitted. The patients assigned to the student will require different services from Progressive Health Care to give the student the most exposure to all of our services. There are written expectations of the student for initial assessment of the patient, weekly follow-up and discharge requirements. Depending on the patient load, the students will get at least 1 home visit to admit a patient. Much of our assessment is obtained from phone conversations with the patient and nurse, so the student will be expected to effectively communicate on the telephone.

Student Roles and Responsibilities
The students roles and responsibilities include:
IV admixture
Patient Admission to include;
Financial reimbursement
Coordinating home care or nursing home
Completing patient chart and write patient orders
Complete pharmacy notes – clinical documentation
Perform MTM
Order necessary labs to monitor therapy
Address patient questions or concerns regarding infusion therapy.
Weekly Patient Monitoring:
          Contact the patient and/or caregiver and nursing agency to monitor the                  patients therapy
           Obtain lab values and make necessary changes to medication therapy.
Discharge Patient
Complete appropriate paperwork and send survey to Patient.
     Presentation to pharmacy staff

Educational Opportunities:
Attend hospice team meetings for 3 hospice agencies in the area.

There are no additional requirements for entrance into this APPE.
