New Practice Experience


Introduction

Dear Potential Preceptor:

Thank you for your interest in developing an advanced pharmacy practice experience site for University of Minnesota College of Pharmacy students.

Enclosed is an Advanced Pharmacy Practice Experience Proposal packet. Please complete the forms and return them to the Office of Experiential Education for review. Send completed forms to:

Forms must be submitted by December 15 be considered a potential training site for the academic year which begins the following May. We recommend that you submit your completed paperwork as soon as possible so we have an opportunity to resolve any potential questions before that date.

A Committee on Experiential Practice reviews Advanced Pharmacy Practice Experience Proposals. The approval process takes 4-6 weeks. After review of your proposal, you will be notified if your proposed Advanced Pharmacy Practice Experience has been approved, or if changes need to be made.

  • You may print the forms and complete them with a typewriter or  pen.
