Application for Admission to the Department of Medicinal Chemistry and Consideration for Graduate Assistantship or Fellowship Form
Instructions for Completing Form:
All application materials for U.S. and international students must be post-marked on or before December 15, 2008.
You have two options for completing this form:
- You may print the form and complete it with a typewriter or an ink pen.
- You may complete the form from within your web browser and print it.
If you chose to complete the form from within your web browser:
- You will need the Adobe Acrobat Reader to access the form. (This free software can be downloaded via this link.)
- You may use either the Tab key or the mouse to move from field to field.
- You must complete and print the entire document before logging out of Adobe Acrobat. Depending on the type of browser you use, you may not be able to save an incomplete form. If you log out before the form is completed, you may lose your work.
