Instructions for Completing the Graduate Applicant Profile
Our Graduate Applicant Profile form is available online. You have two options for completing the form:
- You may download and print the form and complete it with a typewriter or an ink pen.
- You may complete the form from within your web browser and print it.
If you chose to complete the forms from within your web browser:
- You will need the Adobe Acrobat Reader to access the form. This free software can be downloaded via this link.
- You must complete and print the entire document before logging out of Adobe Acrobat. You cannot save an incomplete form. If you log out before the form is completed, you will lose your work.
- To print the form, be sure that the cursor has been clicked outside the text boxes. The information you've entered on the form may not print if the cursor is activated inside any text box.
Instructions for Completing the Supplementary Application Form
The Supplementary Application Form is available online. You will need the Adobe Acrobat Reader to access the form. This free software can be downloaded via this link.
- Please answer the questions contained in the form on a separate piece of paper. Please be sure to include your name on the page containing your answers.
